Portsmouth is Dominica's second town, home to about 3,600 people. For four decades its economy leaned on Ross University's medical campus at nearby Picard, until the school moved to Barbados at the start of 2019 after Hurricane Maria. In 2023 a new school, the American Canadian School of Medicine, opened on the same campus, and the town is still finding its feet.

It suits retirees and remote workers who want a rural, slow-moving coast on a modest budget and don't mind an hour's drive to the capital for bigger errands. If you need specialist care close at hand or a ready-made circle of Americans, look at Roseau first.

Transport in Portsmouth

The center of Portsmouth is small enough to walk for daily errands, with the waterfront running along the bay. Past that, you're relying on minibuses or a car. Privately run minibuses link Portsmouth and Roseau every 10 to 30 minutes during the day along the west coast road, for about EC$10 to EC$12 (around $4) a ride as of April 2026, and the trip takes about an hour. Service thins out after dark and on Sundays. The State Department says ride-hailing apps aren't widely available, so taxis fill the gaps; they aren't metered, so agree on the fare before you get in.

Most people who settle here end up with a car. Dominica drives on the left, the roads are narrow, steep and often wet, and the State Department rates major roads average to poor. You'll need a local temporary driving permit, which rental desks and the Traffic Department in Roseau issue. A few rental agencies work out of Portsmouth itself, with a smaller selection than at the airport.

Douglas-Charles Airport, where most flights land, is about 45 minutes away on the cross-island road. And the Express des Îles ferry from Pointe-à-Pitre, Guadeloupe docks right at Portsmouth pier after about 2.5 hours at sea, which makes the French islands an easy weekend.

Life in a medical-school town without the old school

Community

There's no organized expat scene in Portsmouth. The biggest foreign presence is the student body at the American Canadian School of Medicine, which expected most of its students to come from Canada, the U.S. and the Caribbean, and students have long shaped the town's modest restaurant and bar scene. The town itself is more village than city, so your circle will mostly be neighbors, the market and the fishing waterfront.

Healthcare

Portsmouth has its own public hospital, a smaller one serving the north, while the island's main hospital is in Roseau. Most advanced care means leaving the country. The State Department says care isn't free even at public facilities, providers often want cash up front, and very serious injuries often need medical evacuation, so it strongly recommends evacuation coverage. LGBTQ expats

Dominica's High Court struck down the laws against consensual same-sex relations in April 2024. The State Department notes there are still no laws protecting gay and lesbian people from discrimination, and Human Rights Watch documented prejudice against LGBT people across the Eastern Caribbean as recently as 2018.
