About healthcare in Croatia
Croatia’s universal healthcare system exemplifies solidarity and reciprocity, delivering high-quality care through a mandatory public insurance scheme, obvezno, managed by the Croatian Health Insurance Fund (HZZO). Funded by income-based contributions, it ranks in the top 25% globally, offering modern facilities and English-speaking doctors, especially in cities like Zagreb and Split. The U.S. Embassy notes that while facilities are of Western caliber, resource strain and medicine shortages can occur, with private care often requiring upfront cash payment.
The system has three tiers: obvezno (baseline coverage), dopunsko (co-pay coverage), and dodatno (private supplemental insurance for enhanced services). Government-owned hospitals dominate, but private clinics supplement care, providing faster access. This blend ensures comprehensive coverage, balancing affordability with advanced medical options.
Eligibility & enrollment
Public System (HZZO - Obvezno):
- Who’s Eligible: All residents and citizens; expats on temporary/permanent residency must enroll after 90 days. Digital nomads and EU residents with EHIC are exempt but may opt in.
- Enrollment: Register with HZZO during residency; non-EU expats pay ~€800 upfront (12 months’ back premiums, 2025 estimate), then ~16.5% of income monthly (€80-€230/person).
- Health Card: HZZO card issued post-registration; essential for services.
Private System:
- Expats can add dopunsko/dodatno or secure international plans via brokers, especially if exempt from obvezno.
